The Central Government Health Scheme (CGHS) gives a plastic card called the CGHS card to all beneficiaries. This card contain with a unique ID with photo. This card is mandatory for CGHS free health services. It cannot be transferred from 1candidate to another candidate.
All beneficiaries of the Central Government Health Scheme (CGHS) are provided with a plastic card that contains a ID number and photo to the beneficiary .The cardholder needs to keep their card safe. whenever it required you should produce it for free health services. It can provides comprehensive medical services to all the central government employees.
This scheme operates under the Ministry of Health and Family Welfare services. This scheme is currently covers 27 cities including

Eligibility for the Cghs:-
Every Central government employee family can eligible for this scheme. Family means husband, wife and their children.
This son and daughter before marriage can be eligible for this scheme.
In case of the son is suffering from any permanent disability of any kind of physical and mental disabilities. He is eligible for CGHS benefits even life time. It is the responsibility of the beneficiary to provide information to CGHS
authorities regarding any changes required in the beneficiary database especially related to death, marriage, earning status about the card holder.
Documents required:-
1. Application in prescribed format
2. Proof of residence
3. Proof of stay of dependents
4. Proof of age of son
5. Disability certificate, if any in case of sons aged 25 & above, who would otherwise cease to be a beneficiary.
6. Individual passport size photos of eligible family members
7. Surrender Certificate of CGHS Card while in service ( only in those cases where CGHS Card was issued while in service)
8. Attested copies of PPO & Last Pay Certificate
9. Draft for required amount towards CGHS contribution – in the name of
‘P.A.O., CGHS New Delhi’ in Delhi and in the name of ‘Additional
Director, CGHS' of the city.
The time required for creating new card.
Normally computerized printout of index card is issued on the same day, when you can apply.
CGHS Plastic cards have any one of the following colour strips at the top indicating the category:
Serving Government employees - Blue Pensioners, ex-MPs , Freedom Fighters, etc., - Green
Member of Parliament - Red
Beneficiaries of Autonomous Bodies / Journalist - Yellow
The CGHS card of a serving employee is valid till his/her retirement day. To continue validity on annual contribution pensioner cards, the contribution has to be done one year prior to the completion year. The CGHS card is only valid in CGHS cities. For pensioners living outside the CGHS areas, a regular CGHS card can be obtained from the nearby CGHS city.
